If you have witnessed or experienced unethical practices, manipulation, retaliation, or systemic pressure tied to MDS completion, your story matters.

You are not required to provide your name, employer, location, or any identifying details.

  • No identifying information is requested
  • IP addresses are not intentionally collected
  • Submissions are reviewed manually
  • Details that could identify individuals or facilities are removed prior to publication

Your safety and privacy are the foundation of this project.

What to Include in Your Submission

When writing your story, consider including:

  • Your role (e.g., MDS Coordinator, Nurse, IDT member)
  • The type of facility involved (without naming it)
  • The nature of the concern (pressure to code, altered documentation, retaliation, etc.)
  • How the issue impacted residents, care planning, or outcomes
  • Whether the issue appears systemic or isolated

Do not include:

  • Facility names
  • Personal names
  • Patient identifiers
  • Exact dates or unique events that could identify you
