The MDS Integrity Project

An independent platform exposing fraud, misuse, and systemic failures within the Minimum Data Set (MDS) process in Skilled Nursing Facilities.

Created by industry professions. Built to protect residents, restore integrity, and bring transparency to long-term care.

Mission Statement

The MDS Integrity Project exists to shine a light on practices that undermine patient care, quality measures, and reimbursement accuracy in skilled nursing facilities.

Through education, real-world accounts, and evidence-based analysis, we aim to distinguish honest error from intentional manipulation, and to give a voice to those who have been pressured to compromise their professional integrity.

The Minimum Data Set directly impacts:

  • Patient outcomes
  • Quality Measures (QMs)
  • Quality Improvement Programs (QIPs)
  • Facility reimbursement
  • Regulatory oversight

When MDS data is manipulated or misrepresented, the consequences are real — affecting residents, staff, and public trust.

This project was created because these issues are rarely discussed openly, and those who witness them often fear retaliation.

What We Publish

  • Clear explanations of MDS regulations and expectations
  • The difference between MDS fraud and MDS error
  • Analysis of common high-risk coding practices
  • How improper MDS impacts QMs, QIPs, and PDPM
  • Anonymous, first-hand accounts from industry professionals
  • Advocacy for ethical documentation and resident-centered care

Integrity Should Not Be Optional

Transparency is the first step toward reform.
